AHMED CAPSULAR TENSION SEGMENT

Iqbal Ahmed, M.D., has developed a capsular tension segment (CTS) that effectively supports the capsule, even in the presence of capsular tears, without causing additional zonular damage. This device, which features a fixation hook similar to that on the Cionni Modified Capsular Tension Ring, has no sharp points to tear the capsulorhexis. Surgeons can use one or more segments to support the capsule temporarily or permanently. The Ahmed CTS is still awaiting FDA approval.

MORCHER CAPSULAR TENSION RING

The Morcher Capsular Tension Ring (CTR), which comes in 12.3-mm, 13-mm and 14.5-mm diameters, was the first to receive FDA approval for ophthalmic use in the United States. This PMMA ring supports weak zonules during cataract surgery, centering the capsular bag and providing greater access to the cataract. Surgeons can insert this CTR manually using a Sinskey hook and forceps, or by using a Geuder injector, which facilitates proper and smooth placement by dialing the ring into the capsular bag. Advantages of the Morcher CTR include better IOL centration and overall capsule stability.

CIONNI MODIFIED CAPSULAR TENSION RING

Patients who have significant zonular dialysis or marked capsular decentration often need more support than that offered by basic capsular tension rings (CTRs). To overcome this problem, Robert J. Cionni, M.D., added a horseshoe-shaped fixation hook about 0.25 mm above the tension ring plane. He uses this ring to immobilize the CTR by anchoring the fixation hook to the scleral wall with sutures. Use of the Cionni Modified CTR is limited to patients with intact capsulorhexes and posterior capsules. This device is still awaiting FDA approval for ophthalmic use in the United States.
